Commercial Insurance
Zatkoff Seals & Packings and its subsidiaries and affiliates maintain a comprehensive commercial insurance and risk management program in order to protect our company interests as well as to comply with contractual obligations. To simplify and enhance how we provide evidence of insurance information to our customers, we have provided a link below to our Memorandum of Insurance (MOI) in lieu of manually issuing thousands of individual certificates of insurance. The MOI is available for viewing, downloading, and printing at any time for your convenience.
The MOI gives information about Zatkoff’s insurance programs in the same way as a Certificate of Insurance (COI), but with the added benefit of real-time verification.

FAQs
What is a Memorandum of Insurance?
An MOI provides online information about Zatkoff’s insurance program. This information includes policy numbers and effective dates, limits, insurer identification and applicable enhancements regarding additional insured, waiver of subrogation and primary and non-contributory per the terms of signed contracts, leases, and/or agreements. Like a Certificate of Insurance (or COI), an MOI is an acceptable method to evidence proof of coverage information to other businesses or customers. An electronic MOI eliminates substantial paperwork and processing time. The MOI is regularly updated as the detailed various lines of insurance are renewed annually. Check back at this link whenever you require the latest proof of insurance from Zatkoff.
Additional Insured Status / Waiver of Subrogation
Additional insured and other special insurance requirements are generally conferred on customers where required by written contract, in accordance with the specific insurance policy language. This means that the contract between you and Zatkoff determines additional insured status. This is specific to each insurance policy and is noted in the Additional Information section of the MOI.
Will Zatkoff Still Issue a Certificate of Insurance?
Beginning in 2022, Zatkoff will strongly encourage all customers to rely on the MOI, rather than a paper Certificate of Insurance. The MOI is not only a more accurate real-time status of our insurance policies and specific applicable endorsements, but also the electronic MOI increases the efficiency of your operations by providing your company with more timely insurance information at the click of a link, rather than waiting for a paper certificate to arrive in the mail or email. In certain circumstances, however, we understand a certificate of insurance may still be required. In those cases, please contact your Zatkoff representative with a request for a certificate of insurance along with the specific wording requested for the certificate. Zatkoff’s legal department, along with our insurance broker, will review providing a Certificate of Insurance on a case-by-case basis.

Transparency in Coverage Download
The link above leads to the machine-readable files that are made available in response to the federal Transparency in Coverage Rule and includes negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. The machine-readable files are formatted to allow researchers, regulators, and application developers to access and analyze data more easily.
